Your Support System for a Safe Return Home
Patients who are admitted to the Hospital are supported by an experienced team of case managers and social workers who are dedicated to ensuring that each patient receives high quality care and a safe discharge or check out from the Hospital.
Before leaving the Hospital, a case manager will visit the patient and family members to determine if services will be needed once the patient returns home. These services could include rehabilitation, home care and nursing services to name a few.
Each social worker is licensed and New York State Certified in Social Work to assist patients with social and emotional needs. Plus, each social worker has a masters degree in social work.
Our teams of case managers and social workers are available Monday Saturday to assist you and your family during your stay in the Hospital.
Here are some of the services our teams provide:
- Assessment of patient and family needs
- Counseling and support for:
o Emotional needs o Crisis situations o Coping and adjusting to new illness and treatment o Grief and bereavement o Alcohol and drug use/abuse o Legal issues, such as adoption and custody o End of life issues o Financial needs o Educational and vocational needs
- Advocacy for patients and families
- Assessment of abuse and neglect, domestic violence, physical assaults, and rape or other sexual assaults
- Information and referrals to community services and agencies
